Course Description
Restorative and cosmetic treatment is more esthetic and durable when executed on patients with optimal overbite, overjet and incisor angulation. Dentofacial orthopedics can provide the foundation necessary for restorative dentists to provide their best care. The majority of malocclusions in the United States are Class II malocclusion with mandibular retrusion and excessive overjet. Proper treatment in adolescence can provide the optimal foundation for restorative and cosmetic treatment in adulthood. This lecture will show how a Herbst Appliance is the treatment of choice for correcting Class II malocclusions when increasing mandibular projection is desired and illustrate other methods a Herbst Appliance can be used to treat mandibular asymmetries, resolve maxillary crowding, and correct Class II malocclusions in non-growing patients.
I have given this lecture to orthodontic residents, and I have been invited to present this lecture at the 2026 American Association of Orthodontists meeting in Orlando this May. I am honored to have the opportunity to deliver this lecture to the Mount Baker District Dental Society.
Course Objectives
1. Learn why the Herbst Appliance should be used preferentially in the treatment of Class II malocclusions when an increase in mandibular projection is desired.
2. Discover how a Herbst Appliance can be used to assist the treatment mandibular asymmetry, Class II malocclusions in non-growing patients, and maxillary crowding.
3. Learn an innovative treatment protocol for closing the space of missing mandibular premolars while simultaneously correcting a Class II malocclusion.

About Dr Cummins
David M Cummins received his DDS from Indiana University in 1986. He worked as a Dental Officer in the United States Navy for 3 years and a contract dentist in a private practice for one year before attending the Graduate Orthodontic Program at the University of Iowa. He received an MS in Orthodontics in 1992 and his Master's Thesis generated six peer reviewed papers published in the American Journal of Orthodontics and Dentofacial Orthopedics and The Angle Orthodontists, with one winning the B.F. and Helen E. Dewel Award. Dr Cummins established his orthodontic practice in Bellingham, WA in 1992 where he has practiced for 32 years. He is a Diplomate of the American Board of Orthodontics, Fellow of the American College of Dentists, and a member of the Pierre Fauchard Society. His orthodontic interests are evidence-based Dentofacial Orthopedics, and he has been a guest lecturer on the use of the Herbst appliance at the orthodontic residencies of Indianan University and the University of Iowa and will be presenting his lecture on The Versatile Herbst Appliance at the 2026 American Association of Orthodontist meeting in May. Dr. Cummins has also developed a technique for closing the spaces of missing mandibular premolars in full step Class II malocclusions, eliminating the need for implants an crowns, which was published in a 2018Ormco/AOA Case Studies manual.
